Carolyn Streaker has worked in nursing since 2002. She knew she wanted to work with the senior population early on, minoring in Gerontology while earning her bachelor’s in nursing from the University of Central Florida. She started her career in acute care rehabilitation for spinal cord injury, then as a Nurse Educator in transitional care and med/surgery and soon found her passion in the MDS Coordinator role, where she became nationally certified. She earned her master’s degree in nursing, Clinical Nurse Leader Track, which focused on quality, leadership, and outcomes measurement. Eventually, she brought her leadership and MDS experience to community based Skilled Nursing Facilities, and then moved into consulting and regional Clinical Reimbursement management, where she earned her Advanced certification for Resident Assessment Coordinators through AAPACN.
Carolyn inspires others to strive for the highest level of quality in the Skilled Nursing Facility. She enjoys teaching the interdisciplinary team members about how their input into the MDS assessment contributes to the overall picture of quality and reimbursement outcomes.

When not at work Carolyn enjoys spending time with her husband and her son, reading and baking.
